PCRS Rating Forms

Three rating forms are available: Patient, Relative, and Clinician. The Patient form plus at least one of the other forms must be used concurrently in order to produce a discrepancy score. The items on the three forms are identical except for appropriate wording changes. The Relative and Clinician form also have spaces to indicate the relationship to the subject and a rating of how well acquainted the informant is with the subject's behavior
